We are experienced Assisted Living and Memory Care Registered Nurses who help seniors and families navigate transitions into Independent Living, Assisted Living, and Memory Care Communities. Seniors and families are able to utilize our full range of services, or we can tailor a plan to meet your specific priorities and budget.
Our background in Assisted Living and Memory Care Nursing allows us to customize questions for each Community that are important for seniors and families to ask with preferences, present health status, and future healthcare needs in mind. The goal is to help match seniors and families with Communities that will enable them to age in place, avoid the need for additional transitions, and receive the social engagement opportunities that bring the most enjoyment within a physical environment that is the most comforting. Back Nine Nurses do not receive commission fees from any Communities.
We provide free consultations and help align you with the services and supports that you need until you are ready for a move into Independent Living, Assisted Living, or Memory Care.

Case Management Professionals
|
This includes coordinating medical services, making home visits, identifying legal and financial resources, advocating for elders, and providing reassurance and support to families who can be overwhelmed with caregiving or managing the care of a loved one from a distance. We develop a Care Plan with a thorough assessment of needs, including a realistic evaluation of the elder’s functioning within their environment, and an understanding of their physical, cognitive, financial, and social well-being, along with information from key providers. This Care Plan addresses both short- and long-term goals, and recommendations are made for services that will assist the elder in reaching those goals. Case Management Professionals will then arrange and coordinate all needed services in the home, community, or care facility, and monitor them on an ongoing basis. Family contacts and consultation continue as needed to provide a reliable link to local or long-distance caregivers. |

This could include in-home care, home therapists, lab testing, screening appointments, vaccination recommendations, medical appointments, and more. This can, and often does, include rides to and from necessary medical and preventative care appointments. The goal of care coordination is to advocate for a person’s overall quality of life by streamlining the number of appointments while still allowing that person to get the quality of care they deserve and desire. It’s also support through transitions if an ER or hospitalization is needed, or a move from home to assisted living is wanted. Reliable RN is committed to putting the health back in healthcare through support, advocacy and care. Reliable RN strives to help individuals and families promote better health and wellness at home, minimizing the need for urgent care, ER, and hospital visits. Reliable RN also assists families as they navigate through transitions of care, working to make those transitions as seamless and safe as possible.
