This includes coordinating medical services, making home visits, identifying legal and financial resources, advocating for elders, and providing reassurance and support to families who can be overwhelmed with caregiving or managing the care of a loved one from a distance. We develop a Care Plan with a thorough assessment of needs, including a realistic evaluation of the elder’s functioning within their environment, and an understanding of their physical, cognitive, financial, and social well-being, along with information from key providers. This Care Plan addresses both short- and long-term goals, and recommendations are made for services that will assist the elder in reaching those goals. Case Management Professionals will then arrange and coordinate all needed services in the home, community, or care facility, and monitor them on an ongoing basis. Family contacts and consultation continue as needed to provide a reliable link to local or long-distance caregivers. |
