Our background in Assisted Living and Memory Care Nursing allows us to customize questions for each Community that are important for seniors and families to ask with preferences, present health status, and future healthcare needs in mind. The goal is to help match seniors and families with Communities that will enable them to age in place, avoid the need for additional transitions, and receive the social engagement opportunities that bring the most enjoyment within a physical environment that is the most comforting. Back Nine Nurses do not receive commission fees from any Communities.
We provide free consultations and help align you with the services and supports that you need until you are ready for a move into Independent Living, Assisted Living, or Memory Care.

Case Management Professionals

This includes coordinating medical services, making home visits, identifying legal and financial resources, advocating for elders, and providing reassurance and support to families who can be overwhelmed with caregiving or managing the care of a loved one from a distance. We develop a Care Plan with a thorough assessment of needs, including a realistic evaluation of the elder’s functioning within their environment, and an understanding of their physical, cognitive, financial, and social well-being, along with information from key providers. This Care Plan addresses both short- and long-term goals, and recommendations are made for services that will assist the elder in reaching those goals. Case Management Professionals will then arrange and coordinate all needed services in the home, community, or care facility, and monitor them on an ongoing basis. Family contacts and consultation continue as needed to provide a reliable link to local or long-distance caregivers. |
