Our Mission & History

For over 50 years, Elder Services of Cape Cod and the Islands has been finding creative solutions to ensure we can all thrive as we age. We are your connection to the resources you need to stay safe, healthy, and independent.

Elder Services of Cape Cod and the Islands, Inc. is a nonprofit, community-based organization dedicated to promoting welfare, enhancing the quality of life, and maintaining the dignity of elders in Barnstable, Dukes (except Gosnold), and Nantucket Counties. The organization acts as a central source for information and referral, as an advocate, collaborator, and catalyst, as a coordinator of services and care, as an educator, and as a provider of direct services.

Elder Independence is Our Focus

Our in-home and community-based services are available to all who meet the eligibility criteria without regard to race, ancestry, national origin, language, religion, gender identity, or sexual orientation.

  • Home Care Program
    o Provides supportive services that will help an elder remain safely in the community. To qualify, individuals must be sixty or older, meet financial eligibility, and have specific unmet care needs. Learn more
  • Senior Nutrition Program
    o If an elder is homebound and unable to prepare a meal, they can receive Meals on Wheels delivered to their homes. Learn more
  • Long-Term Care Screening
    o Our Health Services Unit provides required screening for individuals seeking Medicaid reimbursed services such as nursing home care, adult day health programs, or other community-based services. Learn more
  • Options Counseling
    o Options Counseling is a short-term decision support process to assist consumers of any income (elders age 60 or older, or individuals over the age of 18 with any disability) and their family members, caregivers, and/or significant others in making an informed choice about long term services, supports, and setting.   • Family Caregiver Support Program
    o Strives to support and promote the ability of caregivers to continue in their caregiving role by helping them to make informed decisions, solve problems, and reduce stress. Learn more
  • Money Management Program
    o Money Managers assist income-eligible seniors aged sixty or older who may be at risk of losing their independence due to their failure to pay rent, utility, or other basic bills on time. Learn more
  • Congregate Housing at Hansen Village
    o Congregate housing offers affordable and supportive shared living arrangements for adults over 60 or living with a disability as an alternative to premature nursing or rest home placement. Learn more
  • Senior Care Options (Managed Care for MassHealth Enrollees)
    o The Senior Care Options (SCO) program is a Mass Health Program that covers all services reimbursable under Medicare and MassHealth through a senior care organization and its network of providers. The goal of the Senior Care Options programs is to help their consumers remain in the community as independently as possible. Learn more
